• Parent Organization's FEIN (Federal Employer Identification Number)

• IRS Determination Letter (PDF Format)

• Name, address, and phone number of both the Parent Organization and the Sub-Organization applying for the grant

• NOTE: Only PDF formatted files are accepted (yes, pictures must be in PDF format as well).

  IMPORTANT:

• 1. Once you begin the registration process, please complete all steps to ensure your account is created successfully.

• 2. Draft application status will expire after 15 business days. Once it expires, you must resubmit your application.
